The following document is intended to form an agreement for a professional Counselling relationship between the two parties outlined below. This contract is subject to re-negotiation at any stage of the working relationship by agreement of both client and therapist.
cambridge weight plan®: Refund of Cambridge products are at discretion and can incur a £25 charge for each consultation where products have not been purchased or have been returned. Items ordered in at clients' request cannot be returned.
cancellations: All missed appointments are payable at the usual fee. Appointments cancelled with less than 48 hours notice are also charged at the session fee rate. We’re happy to reschedule your appointment with at least 48 hours notice.
client correspondence: All 'Client' correspondence will be responded to as a matter of priority. Should investigation be deemed necessary, a maximum of 28 days response time, in the first instance may be required.
confidentiality & ethical practice: It is agreed that the content of our work together will be conducted in accordance with the BACP Ethical Framework for Good Practice in Counselling and Psychotherapy. However, should during the course of our work together I consider that you are likely to endanger or cause harm to either yourself or another person I retain the right to consult or inform an outside authority such as for example, your GP or the Police. I will endeavour to inform you of my decision and explain my reasons why prior to carrying out this action. However, I retain the right to do so without prior consultation with you should I consider that the urgency of the situation requires me to act immediately to safeguard the physical safety of yourself or others.
efficacy: The efficacy of all therapies is dependant on individual client responses.
late arrival: It is expected that the session will begin at the agreed time. Any session that begins after this time due to late arrival for whatever reason cannot be extended beyond the agreed finish time.
method of payment: By cash, visa or credit card at the conclusion of each session for private individuals. Payments by invoice for Employee Assistance Programme (EAP) company/agency schemes etc. Pre-paid clinical sessions cannot be refunded, however alternative appointments can be made at your convenience.
practice location: Temple Court, 13a Cathedral Road, Cardiff. CF11 9HA.

referral: It may also be possible that our work together may have successfully highlighted the need for me to recommend a referral to another counselling/psychotherapist practitioner for some form of specialist therapy, beyond the scope of my professional training and experience. Alternatively it may (for example) include a referral to your GP for possible medication/or GP referral to another NHS consultant.
record keeping: The service offered is confidential and brief notes are kept during therapy. Paper records are destroyed after 6 months from your last appointment in line with BACP guidelines. Electronic information is kept on file of your contact details and brief reason for initial visit, should you attend future sessions. Client email addresses are entered into a secure newsletter database managed by the Limited company and can be opted–out at any time. No information is shared or sold with any marketing organisation.

review of fees: The level of fees is reviewed once per year and any changes take affect from January 1st. If any changes are deemed necessary they will only apply to new or renewed contracts. Cambridge Weight Plan products can increase at anytime throughout the year in line with increases in manufacturing costs.
sessions: Each clinical session lasts for 50 minutes. The amount of sessions required is dependant on each individual case and this will be discussed with the client at the first session and can be reviewed throughout the process.
supervision: Clinical supervision is conducted in accordance with the BACP guidelines for supervised client-work and is attended by Andy Garland on a monthly basis.
therapy offered: All therapeutic interventions are offered with the discretion and professional knowledge of the therapist. Where therapy is deemed inappropriate, this will be discussed with the client and alternative suggestions may be made.
training: Terms relating to training bookings will be sent to you with your invoice.
